January 20, 2021: If you have registered to receive your COVID-19 vaccination under the Tier B category or know someone who has, please complete the Vaccination Administration Form and bring it with you to your scheduled appointment.
Do not register for your vaccination in more than one place .
If you have, cancel the other locations once you have received your vaccination to minimize confusion with distribution and ensure the timeliness of others receiving their vaccination.
